Does your child need glasses? ASCC can help.
The purpose of this program is to reimburse up to $100 of the cost of eyeglasses for qualifying school-age children in Ansbach/Katterbach schools. In order to qualify, each child must be a dependent/reside in the USAG Ansbach area .
In order to apply for funding through Bright Eyes, please do the following
1. Pick up a Bright Eyes Referral Form from your school nurse. Please fill out the top portion of the form and have the school nurse sign the form prior to your optometry clinic appointment.
2. Make an appointment with the Optometry Clinic. Make sure to bring your Referral Form to your appointment. If the Optometrist determines that your child needs glasses, ensure that he/she fills out the Optometry portion of the Referral Form. The Optometrist should also staple the prescription to the Referral Form.
3. Purchase your child’s eyeglasses. Bring the prescription and paperwork to an eyeglass vendor of your choice. Make sure that the salesperson fills out the portion of the Referral Form indicating the cost of the frames, lenses, and total cost. Costs may be written in Euro or in US Dollars.
4. If your child requires special lenses or frames due to special medical conditions, please discuss this with the ASCC Welfare Chairperson BEFORE purchasing your child’s eyeglasses. ASCC will assess the situation and make a determination as to whether or not additional funds are warranted.
5. Submit the completed Referral Form, prescription, and receipt for purchase of eyeglasses to the following address:

ASCC will reimburse you up to $100 of the purchase price of your child’s eyeglasses. ASCC will mail your reimbursement check via the APO address you provide on your Referral Form.
